In 1919, Wertheim opened a practice for venereal diseases and for counseling former venereal disease patients within the Institute for Sexual Science. He collaborated with Hirschfeld and Kronfeld on expert reports to determine paternity and with regard to alimony claims. He had access to a laboratory for urine, sperm, and blood tests for his examinations.

Wertheim left the Institute in 1921 and opened a private medical practice in Berlin.
